UHIP Claims Process

 

Every time you access healthcare, a claim form is completed.

Generally, when you visit a Preferred Provider they will complete the form on your behalf and send it directly to the insurer for payment.

The health care providers noted on our Local Kingston Health Services page are all considered Preferred Providers.

If you are required to pay for health services at the time of the visit (or if you receive a bill at a later date) you will need to send the following to the insurer to be reimbursed for any eligible costs:

  • a complete a claim form, with;
  • the original invoice/receipt.

Claim forms are available from the UHIP website. A tutorial is available to assist you in completing the claim form.
